On August 31, 2026, Goethe University Frankfurt and Universitätsmedizin Frankfurt published findings showing that seminal fluid actively facilitates monkeypox virus entry into human cells — a mechanistic explanation for something epidemiologists have observed for years without being able to explain. The study, led by Professor Denisa Bojkova with experiments conducted by PhD student Emma Torbica, appeared in Emerging Microbes & Infections under the title "Seminal fluid enhances monkeypox virus infection via amyloidogenic fibrils."

For sexual health services and the STI-testing sector, this is the kind of finding that shifts prevention messaging from correlation to mechanism. "Sexual contact spreads mpox" was already established; "seminal fluid is an active infection enhancer, and undetected mucosal lesions are the entry point" is a materially more actionable statement. It strengthens the case for barrier methods specifically — not just partner-count reduction — and for vaccination in high-exposure groups. The prevention-technology angle is the more interesting commercial one. If an experimental compound can disrupt amyloidogenic fibrils in seminal fluid, that is in principle a topical or lubricant-adjacent intervention rather than a systemic drug. The sexual wellness industry has spent years trying to build credible antimicrobial or virus-inhibiting lubricants, mostly without clinical substance behind the claims. A fibril-disrupting agent with published mechanism data is a genuinely different proposition — though it remains preclinical, and no product exists. There is also a broader signal for how mpox is spreading. Lageso's observation of transmission in mixed heterosexual and queer settings undercuts the framing of mpox as an exclusively MSM concern, which has consequences for who gets offered vaccination and who thinks to get tested. Public health messaging that has not updated since 2022 is now measurably behind the epidemiology.

The experimental design was direct. Researchers infected connective tissue cells (fibroblasts) and epidermal cells (keratinocytes) with monkeypox virus. When the virus had first been mixed with seminal fluid, uptake into the cells increased. Analysis of the fluid identified the culprit: naturally occurring protein fragments that self-assemble into fibers, which then act as a bridge helping the virus attach to human cells. This is the same class of mechanism — semen-derived amyloid fibrils — that virologists documented decades ago as an enhancer of HIV infection, now demonstrated for a second, structurally unrelated pathogen.

"Our findings provide an explanation for why sexual contact carries a particularly high risk of transmission," Torbica said. "Seminal fluid can therefore promote infection when, for example, it comes into contact with anal or genital mucous membranes where pox-like lesions have already formed but have not yet been noticed." That last clause matters clinically: mpox begins with nonspecific symptoms — fever, headache, joint pain — and the characteristic lesions can appear late and on mucous membranes where they go unseen.

The study also carries treatment findings. Bojkova reported that available antiviral drugs remained effective under these enhanced-infection conditions, and that an experimental compound was able to disrupt the infection-promoting fibril structures while directly inhibiting the virus as well. "In the long term, this could open up new approaches to prevention," she said.

Context on why anyone should care: the WHO has twice declared mpox a public health emergency of international concern, most recently in 2024. A variant that emerged in 2023 infected roughly 100,000 people across several African countries in the first half of 2024 alone, killing more than 200 — primarily children and immunocompromised individuals. Cases continue at low levels in Germany, and Berlin's state health authority (Lageso) has recently reported transmission increasingly occurring through sexual activity between men and women, including cases linked to sex-positive events for both heterosexual and queer participants. The Robert Koch Institute describes the available vaccine as highly effective and well tolerated.
